Facilitate development of Circle of Support.

Reviews all collateral material obtained at admission and on an ongoing basis.

Complete weekly visits to client's homes.

Initiate and implement emergency assistance, responsive, system for clients.

Provide assistance as needed to locate an appropriate roomate situations for client on caseload as well as assistance with moving.

Complete required paperwork (i.e. monthly summaries and performance evaluations).

Facilitate communication and teamwork among client and team (including but not limited service coordinator, Families, landlords, and etc).

Other duties assigned by Community Service Facilitator or Open Door SLS owners.
As a facilitator, your primary responsibilities include guiding group discussions, maintaining a productive atmosphere, and ensuring effective communication. You’ll prepare agendas, manage discussions, and select relevant materials. Additionally, you’ll assess participants’ progress, balance facilitation with teaching, and identify capacity development needs. Strong communication skills, adaptability, and experience in facilitation are essential for success in this role.
